Privacy Complaints

If an individual feels that his or her personal information has been collected, used or disclosed by a government organization in contravention of the Freedom of Information and Protection of Privacy Act (FIPPA) or the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA), he or she should call, write or visit the government office that has possession of the information to try to resolve the matter. If an individual feels that his or her personal health information has been collected, used or disclosed by a health information custodian in contravention of  the Personal Health Information Protection Act (PHIPA), he or she should call, write or visit the custodian that has possession of the information to try to resolve the matter.  If the matter cannot be resolved with the government organization or the health information custodian, the individual may contact the IPC to file a privacy complaint.

To file a privacy complaint under FIPPA or MFIPPA, you may fill out the Privacy Complaint FormThis form must be printed and mailed to the IPC Registrar, at the address on the form, who will open a complaint file and assign an intake analyst to proceed with the complaint. The complaint process under FIPPA or MFIPPA is outlined in our Privacy Complaint Process Flow Chart.

To appoint another person (other than a lawyer) to act as your agent during the complaint process under FIPPA or MFIPPA, you may wish to fill out the form Authorization to Act as an Agent for Privacy Complaint.
