Annual Statistical Reporting
These forms are to be used to report on requests for access to and/or correction of information and/or personal health information for the 2009 calendar year. February 1, 2010 is the deadline to complete and return the statistical reports to the IPC.

For your convenience you may want to complete and submit an online report at https://statistics.ipc.on.ca or it can be faxed to us at (416) 325-9195 or mailed to the Office of the Information and Privacy Commissioner/Ontario, 2 Bloor St. E., Suite 1400, Toronto, ON M4W 1A8.
