Summary  Notice to Hospitals

NOTE: 2012 Online Statistical Reporting is now open. The deadline for reporting is March 1, 2013. 


Institutions that fall under the provincial or municipal Freedom of Information and Protection of Privacy Act are required by these laws to report to the IPC each year:


  • the number of written requests for access under FIPPA/MFIPPA and the Personal Health Information Protection Act (PHIPA);
  • the number of refusals under FIPPA/MFIPPA and PHIPA, the provisions under which the refusal was made and the number of occasions on which each provision was invoked;
  • the number of uses or purposes for which personal information, including personal health information, is disclosed where the use or purpose is not included in the personal information bank index under FIPPA/MFIPPA or the written public statement under PHIPA;
  • the amount of fees collected; and
  • other information indicating the effort to put into practice the purposes of these statutes.

Institutions are required to submit a report even if they have had no requests during the 2012 calendar year.

March 1, 2013 is the deadline to complete and return the statistical reports for 2012 to the IPC.
