MAKING A COMPLAINT:
Collection, Use and Disclosure Complaint Form
A form for filing a complaint under the Personal Health Information Protection Act, if you feel your personal health information has been improperly collected, used or disclosed by a health information custodian. The form can be completed online and then printed, or printed and then filled out with a pen. The completed form is then sent to the IPC. Because of privacy and security concerns, the form cannot be submitted online. Use of the form is optional, as complaint letters are still accepted. (In PDF format)
Authorization to Act as an Agent For A Complaint under the Personal Health Information Protection Act
A form to be signed by a complainant who is represented by someone other than a lawyer.
