MAKING A PRIVACY COMPLAINT:
Your Privacy and Ontario's Information and Privacy Commissioner
Provides answers to questions most frequently asked about personal privacy and government-held information.
Privacy Complaint Form
A form for filing a privacy complaint, which can be completed online then printed, or printed then filled out with a pen. The completed form is then sent to the IPC (the form cannot be submitted online). Use of the form is optional, as appeal letters are still accepted. (In PDF format)
Authorization to Act as an Agent for Privacy Complaint
A form to be signed by a complainant who is represented by someone other than a lawyer in a privacy complaint. (In PDF format)
