PHIPA Practice Direction #2: Responding to a Request for Access to Personal Health Information

In Ontario, individuals have a right to request access to their health information pursuant to the Personal Health Information Protection Act. This Practice Direction compliments the detail within PHIPA Practice Direction #1: Clarifying Access Requests, and provides additional clarification to assist health information custodians when they are responding to an individual’s access request(s). Appended are samples of a checklist, a response letter and an index of records.
